This is a multi-step process so here is what you can expect:
Gather all required documentation and upload individually for review

Documents should be scanned and uploaded in a .pdf format but if you are unable to do so, taking a photo with a smart device and uploading them as a .bmp, .gif, .jpeg or .jpg, .png, or .tif image file is acceptable as well.
An email is generated to you that the document(s) needs approval
A benefits management representative will review them
If anything is missing, your agency human resources office will contact you to upload the missing documentation
If everything is uploaded correctly, the documents will be approved
You will receive a second email to log back into myOhio and complete the enrollment process
You are not done until you 1) submit your elections and 2) click Complete two pages later
The changes to your health benefit elections will be effective July 1, 2023. The applicable third-party administrators, e.g., Medical Mutual or Anthem and Optum Rx, are notified of the enrollment change in early June.

Below is a list of acceptable secondary documents to verify the eligibility of a spouse for benefits coverage:

Property Tax Statement
Bank Statement
Credit Card Statement
Auto Loan / Insurance Statement
Please note that separate statements with the employee’s name and another in the spouse’s name is acceptable as proof of an ongoing marriage if the statements are dated, or reflect a due date, within the last twelve months, and indicate the same mailing address. For example, the following combination of documentation is acceptable for verification purposes:
