This topic gives instruction when electing Family coverage from that of Waived or Single coverage. The instructions are the same if you already have family coverage and wish to add additional dependents.
The year found in dates referencing Open Enrollment periods as well as specific benefit contributions have been masked as these instructions are applicable regardless of year and contributions may change from year to year.
Also, not all plans may be displayed in the job aid and accompanying simulation as plans may be added or removed from year to year. Please refer to your myBenefits Guide for specific plan and contribution information.
Steps:
Begin the Benefits Enrollment Opportunity from the myOhio Homepage.
Click the MY WORKSPACE tab.
Step 1The MY WORKSPACE page displays.
Click the myBenefits link.
Step 2The OAKS Benefits navigation collection displays on the left.
Select the Benefits Enrollment menu item.
Step 3Click the Navigation Area grip.
Step 4The Benefits Enrollment page displays.
Click the Start button.
Step 5The Welcome to The State of Ohio Open Enrollment page displays.
Click the Next button.
Step 6The Dependent/Beneficiary Info page displays. From here, you may add additional dependents.
Click the Add Individual button.
Step 7Begin by adding the dependents name.
Click the Add Name button.
Step 8This field is optional.
Click the Name Prefix list.
Step 9Click the Mrs list item.
Step 10Click in the First Name field.
Step 11Enter the desired information into the First Name field. Enter
Click in the Last Name field.
Step 13Enter the desired information into the Last Name field. Enter
Click the Name Suffix list.
Step 15With the dependent's name information entered, click the Done button.
Step 16You may either enter the date directly in the field or use the calendar icon to select a date.
Click in the Date of Birth field.
Step 17Enter the desired information into the Date of Birth field. Enter
Click the Gender dropdown button to activate the menu.
Step 19Click the Female list item.
Step 20Click the Relationship to Employee dropdown button to activate the menu.
Step 21Click the Spouse list item.
Step 22Click the Marital Status dropdown button to activate the menu.
Step 23Click the Married list item.
Step 24You may either enter the date directly in the field or use the calendar icon to select a date.
Click in the As of field.
Step 25Enter the desired information into the As of field. Enter
Proceed down the page adding additional information. The dependent's National ID is next.
Click the Add National ID button.
Step 27Click in the National ID field.
Step 28Enter the desired information into the National ID field. Enter
After entering the dependent's Social Security Number click the Done button.
Step 30Click the Add Phone button.
Step 31The Phone Number pop-up window displays.
Enter the appropriate information. Setting the Same as mine switch to Yes will populate the dependents Phone Number information with the same information that appears in your profile.
Click the Type list.
Step 32Click the Mobile list item.
Step 33Click in the Number field.
Step 34Enter the desired information into the Number field. Enter
With the information entered, click the Done button.
Step 36Click the vertical scrollbar.
Step 37Click the Add Email button.
Step 38The Email Address pop-up window displays.
Enter the dependent's email information. Fields with an asterisk [ * ] are required.
Click the Email Type list.
Step 39Click the Home list item.
Step 40Enter the desired information into the Email Address field. Enter
Click the Done button.
Step 42Click the Save button.
Step 43The Dependent/Beneficiary Info section updates to show the newly added individual.
If additional dependents need to be added, such as children, click the Add Individual button and follow the previous steps for each.
Click the Add Individual button.
Step 44Click the Add Name button.
Step 45Click in the First Name field.
Step 46Enter the desired information into the First Name field. Enter

Step 65The Open Enrollment - Document Upload page displays.
On this page, you will see a listing of valid documents needed to support your election. Documents should be scanned and uploaded in a .pdf format but if you are unable to do so, taking a photo with a smart device and uploading them as a .bmp, .gif, .jpeg or .jpg, .png, or .tif image file is acceptable as well.
Click the vertical scrollbar.
Step 66Click the Add Attachment button.
Step 67The Document Definition - New Attachment page displays.
Click the Description dropdown button to activate the menu.
Step 68Select the appropriate document description.
Click the Marriage Certificate list item.
Step 69Click the Add Attachment button.
Step 70The File Attachment pop-up window displays.
Click the Browse button.
Step 71Your browser's upload pop-up window displays. This window may look different depending on the browser you are using.
From this window select the document you wish to upload.
Click the Marriage Certificate.docx list item.
Step 72Click the Open button.
Step 73The File Attachment pop-up window displays.
Click the Browse button.
Step 74The Attachment is now listed, but must be saved to the enrollment.
Click the Save button.
Step 75Your attachment must be approved by your agency human resources office before you may proceed with your enrollment.
This message is advising you that notification has been sent to your agency human resources office, and that you will receive an email in return when the document has been accepted or declined.
Click the OK button.
Step 76Click the vertical scrollbar.
Step 77Notice the document is listed in the Attachments section at the bottom of the page and a Status of Needs Approval displays.
Repeat the process for all documents needed for the enrollment.
Click the Add Attachment button.
Step 78Click the Description dropdown button to activate the menu.
Step 79Click the Birth Certificate list item.
Step 80Click the Add Attachment button.
Step 81Click the Browse button.
Step 82Click in the Name field.
Step 83Click the Open button.
Step 84Click the Upload button.
Step 85Click the Save button.

Step 88After you have submitted the required documents, exit Open Enrollment. Your agency human resources office will review your attachments for correctness. Please allow 24 hours for review.
Click the Exit button.
Step 89A confirmation window displays advising you that you will return to where you left off in the enrollment process.
Click the Yes button.
Step 90You are returned to the Benefits Enrollment page.
Step 91You will receive an email similar to the one shown once documentation has been approved by your agency human resources office.
Conversely, if the documentation is not sufficient an email will be sent to you advising you of such.
Step 92Continue your Open Enrollment Event from the myOhio Homepage.
Click the MY WORKSPACE tab.
Step 93The MY WORKSPACE page displays.
Click the myBenefits link.
Step 94Click the Benefits Enrollment object.
Step 95The Benefits Enrollment page displays.
Click the Navigation Area grip.
Step 96Click the Start button.
Step 97When you re-enter the Enrollment process you are returned to the step where you left off.
Notice that with your documents approved, the step progress displays as Complete.
Click the Next button.
Step 98The Benefits Enrollment page displays.
Click the Medical Plan Review button.
Step 99The Medical Plan Enrollment page displays.
Note: Available plans to choose from may change from year to year. The plans shown in this job aid may differ from those being offered during the current Open Enrollment period.
Please refer to the MyBenefits Guide for a description and pricing of available plans.
Step 100The information icon provides cost for the plan based on Single, Family No Spouse, and Family with Spouse selections.
Note: the rates depicted are an example and not the actual rates. Please consult your MyBenefits Guide for the actual rates.
Click the Information icon.
Step 101Review the Ohio Med PPO Medical Mutual pop-up window to see the applicable cost to you.
Click the Close [ X ] button.
Step 102To enroll your dependent(s) select the checkbox appearing to the left of their name.
Select the dependent's checkbox.
Step 103Select the dependent's checkbox.
Step 104When all applicable dependents have been selected click the Done button. Please note, this does not complete your enrollment. There are additional steps which must be performed to complete enrollment.
Click the Done button.
Step 105Having reviewed your enrollment elections, you may now submit your enrollment.
You are returned to the Benefits Enrollment page; this page displays your costs.
Notice the Plan Type Status now indicates Changed.
Note: This example does not show the employee is eligible for the dental or vision plans. If you are eligible and wish to enroll or change your elections, please repeat steps 45 – 49.
Note: This is the required step to ensure your selections are conveyed for processing.
Click the Submit Enrollment button.
Step 106A Benefits Alerts pop-up window displays. From here, you may view your Election Preview statement.
Click the View button.
Step 107Review your submitted enrollment. You may view more information using the section expand area.
Click the Close [ X ] button for the window; please do not close the browser window yet as you are not finished.
Step 108You are returned to the Benefits Enrollment page.
Click the Next button.
Step 109This step is optional as you have already viewed the Benefits Statement page from the Benefits Alerts pop-up window.
If you would like to again view the Enrollment Preview, or perhaps print it for your records, click the arrow at the end of the row.
Step 110Click the Next button.
Step 111The Summary page displays.
Click the Exit button.
Step 112A confirmation pop-up window displays.
Click the Yes button.
