Enrollment in Single Coverage


Concept

This topic gives instruction for enrollee's who previously waived coverage and are electing PPO Medical coverage. The steps are the same if you are currently enrolled in family coverage or the high-deductible plan with single coverage.

 

The year found in dates referencing Open Enrollment periods as well as specific benefit contributions have been masked as these instructions are applicable regardless of year and contributions may change from year to year.

 

Also, not all plans may be displayed in the job aid and accompanying simulation as plans may be added or removed from year to year. Please refer to your myBenefits Guide for specific plan and contribution information.


Steps:

  1. Begin the Benefits Enrollment Opportunity from the myOhio Homepage.

     

    Click the MY WORKSPACE tab.

    Click the myBenefits link.

  3. The OAKS Benefits navigation collection displays on the left.

     

    Select the Benefits Enrollment menu item.

  4. The Benefits Enrollment page displays.

     

    Click the Start button.

  5. The Welcome to The State of Ohio Open Enrollment page displays.

     

    Click the Next button.

  6. The Dependent/Beneficiary Info page displays. From here, you may add additional dependents.

     

    Click the Next button.

  7. The Open Enrollment - Document Upload page displays.

     

    On this page, you will see a listing of valid documents needed to support your election. Documents should be scanned and uploaded in a .pdf format but if you are unable to do so, taking a photo with a smart device and uploading them as a .bmp, .gif, .jpeg or .jpg, .png, or .tif image file is acceptable as well.

     

    Note:  The system requires a document be submitted when an election change is being made. Since you are enrolling in single coverage, the document can be a Word document with the words "I am enrolling in Single Coverage" to satisfy the system requirement.

  8. Click the Add Attachment button.

    Click the Description dropdown button to activate the menu.

    Click the Browse button.

  12. Your browser's upload pop-up window displays. This window may look different depending on the browser you are using.

     

    From this window select the document you wish to upload.

     

    Click the Enrollment Letter.docx list item.

  14. The File Attachment pop-up window displays.

  15. The Attachment is now listed, but must be saved to the enrollment.

     

    Click the Save button.

  16. An Approval is required pop-up window displays.

     

    Your attachment must be approved by your agency human resources office before you may proceed with your enrollment.

     

    This message is advising you that notification has been sent to your agency human resources office, and that you will receive an email in return when the document has been accepted or declined.

     

    Click the OK button.

  17. After you have submitted the required documents, exit Open Enrollment. Your agency human resources office will review your attachments for correctness. Please allow 24 hours for review.

     

    Click the Exit button.

  18. A confirmation window displays advising you that you will return to where you left off in the enrollment process.

  19. You are returned to the Benefits Enrollment page.

  20. You will receive an email similar to the one shown once documentation has been approved by your agency human resources office.

     

    Conversely, if the documentation is not sufficient an email will be sent to you advising you of such.

  21. Continue your Open Enrollment Event from the myOhio Homepage.

     

    Click the MY WORKSPACE tab.

  25. When you re-enter the Enrollment process you are returned to the step where you left off.

     

    Notice that with your documents approved, the step progress displays as Complete.

    Note: Available plans to choose from may change from year to year. The plans shown in this job aid may differ from those being offered during the current Open Enrollment period.

     

    Please refer to the MyBenefits Guide for a description and pricing of available plans.

  28. The information icon provides cost for the plan based on Single, Family No Spouse, and Family with Spouse selections.

     

    Note:  the rates depicted are an example and not the actual rates.  Please consult your MyBenefits Guide for the actual rates.

     

    Click the Ohio Med PPO Medical Mutual Information icon.

  29. Review the Ohio Med PPO Medical Mutual pop-up window to see the applicable cost to you.

     

    Click the Close [ X ] button.

  31. Please note, this does not complete your enrollment. There are additional steps which must be performed to complete enrollment.

     

    Click the Done button.

  32. Having reviewed your enrollment elections, you may now submit your enrollment.

     

    You are returned to the Benefits Enrollment page; this page displays your costs.

     

    Notice the Plan Type Status now indicates Changed.

     

    Note: This example does not show the employee is eligible for the dental or vision plans.  If you are eligible and wish to enroll or change your elections, please repeat for the other plans.

     

    Note: This is the required step to ensure your selections are conveyed for processing.

     

    Click the Submit Enrollment button.

  33. A Benefits Alerts pop-up window displays. From here, you may view your Election Preview statement.

     

    Click the View button.

  34. Review your submitted enrollment. You may view more information using the section expand area.

     

    Click the Close [ X ] button for the window; please do not close the browser window yet as you are not finished.

    If you would like to again view the Enrollment Preview, or perhaps print it for your records, click the arrow at the end of the row.
