This topic gives instruction when electing to Waive coverage.
The year found in dates referencing Open Enrollment periods as well as specific benefit contributions have been masked as these instructions are applicable regardless of year and contributions may change from year to year.
Also, not all plans may be displayed in the job aid and accompanying simulation as plans may be added or removed from year to year. Please refer to your myBenefits Guide for specific plan and contribution information.
Steps:
Begin the Benefits Enrollment Opportunity from the myOhio Homepage.
Click the MY WORKSPACE tab.
Step 1The MY WORKSPACE page displays.
Click the myBenefits link.
Step 2The OAKS Benefits navigation collection displays on the left.
Select the Benefits Enrollment menu item.
Step 3Click the Navigation Area grip.
Step 4The Benefits Enrollment page displays.
Click the Start button.
Step 5The Welcome to The State of Ohio Open Enrollment page displays.
Click the Next button.
Step 6The Dependent/Beneficiary Info page displays. As you are waiving coverage, updates to existing dependents are not necessary.
Click the Next button.
Step 7The Open Enrollment - Document Upload page displays.
On this page, you will see a listing of valid documents needed to support your election. Documents should be scanned and uploaded in a .pdf format but if you are unable to do so, taking a photo with a smart device and uploading them as a .bmp, .gif, .jpeg or .jpg, .png, or .tif image file is acceptable as well.
Note: The system requires a document be submitted when an election change is being made. The document can be a Word document with the words "I want to waive my coverage" to satisfy the system requirement.
Click the vertical scrollbar.
Step 8Click the Add Attachment button.
Step 9The Document Definition - New Attachment page displays.
Click the Description dropdown button to activate the menu.
Step 10Click the Add Attachment button.
Step 11The File Attachment pop-up window displays.
Click the Browse button.
Step 12Your browser's upload pop-up window displays. This window may look different depending on the browser you are using.
From this window select the document you wish to upload.
Click the Waive Enrollment Doc.docx list item.
Step 13Click the Open button.
Step 14The File Attachment pop-up window displays.
Click the Upload button.
Step 15The Attachment is now listed but must be saved in order to submit for approval.
Click the Save button.
Step 16Your attachment must be approved by your agency human resources office before you may proceed with your enrollment.
This message is advising you that notification has been sent to your agency human resources office, and that you will receive an email in return when the document has been accepted or declined.
Click the OK button.
Step 17After you have submitted the required documents, exit Open Enrollment. Your agency human resources office will review your attachments for correctness. Please allow 24 hours for review.
Click the Exit button.
Step 18A confirmation window displays advising you that you will return to where you left off in the enrollment process.
Click the Yes button.
Step 19You are returned to the Benefits Enrollment page.
Step 20You will receive an email similar to the one shown once documentation has been approved by your agency human resources office.
Conversely, if the documentation is not sufficient an email will be sent to you advising you of such.
Step 21Select the Benefits Enrollment menu item.
Step 22The Benefits Enrollment page displays.
Click the Navigation Area grip.
Step 23Click the OAKS Benefits object.
Step 24Click the Next button.
Step 25The Benefits Enrollment page displays.
Click the Medical Plan Review button.
Step 26The Medical Plan Enrollment page displays.
Note: Available plans to choose from may change from year to year. The plans shown in this job aid may differ from those being offered during the current Open Enrollment period.
Please refer to the MyBenefits Guide for a description and pricing of available plans.
Step 27Click the Waive Select button.
Step 28A pop-up window displays a warning about disenrolling dependents (if any).
Click the Yes button.
Step 29After selecting to Waive Medical coverage click the Done button. Please note, this does not complete your enrollment. There are additional steps which must be performed to complete enrollment.
Click the Done button.
Step 30Click the Review button.
Step 31Click the Select button.
Step 32Click the Yes button.
Step 33Click the Done button.
Step 34Click the Review button.
Step 35Click the Select button.
Step 36Click the Yes button.
Step 37Click the Done button.
Step 38You are returned to the Benefits Enrollment page; this page displays your costs.
Notice the Plan Type Status now indicates Changed.
Step 39Having reviewed your enrollment elections, you may now submit your enrollment.
Note: This is the required step to ensure your selections are conveyed for processing.
Click the Submit Enrollment button.
Step 40A Benefits Alerts pop-up window displays. From here, you may view your Election Preview statement.
Click the View button.
Step 41Review your submitted enrollment. You may view more information using the section expand area.
Click the Close [ X ] button for the page; please do not close the browser window yet as you are not finished.
Step 42You are returned to the Benefits Enrollment page.
Click the Next button.
Step 43This step is optional as you have already viewed the Benefits Statement page from the Benefits Alerts pop-up window.
If you would like to again view the Enrollment Preview, or perhaps print it for your records, click the arrow at the end of the row.
Step 44Click the Next button.
Step 45The Summary page displays.
Click the Exit button.
Step 46A confirmation pop-up window displays.
Click the Yes button.
Step 47You are returned to the Benefits Enrollment page.
